What is a dental implant, exactly?

On top sits the abutment, a connector that pokes through the gum, and onto that goes the restoration — a crown for a single tooth, or a bridge or denture for several.

The implant is not glued in. It works because bone biologically bonds to the titanium surface, holding it as firmly as a natural root. That bond is the whole basis of implant dentistry, and it has a name.

What is osseointegration and why does it matter?

Osseointegration is the direct structural and functional connection between living bone and the implant surface. After placement, bone cells migrate onto the roughened titanium and lay down new bone in contact with it over roughly 8–12 weeks. Once that has happened, the implant can bear the full force of chewing — often over 100 newtons on a back tooth.

Because everything depends on this bond, I measure it rather than guess. Implant primary stability at placement is judged by insertion torque (typically aiming for 25–45 Ncm), and I can confirm integration before loading using resonance frequency analysis, which gives an ISQ (Implant Stability Quotient) score — values in the 65–75 range generally indicate an implant ready to be restored. This is why a reputable clinic will not simply screw a crown on the same week; the timing follows the biology.

How does the procedure work in 2026?

A typical single-implant journey looks like this:

  1. Assessment and 3D imaging. A cone-beam CT scan shows bone volume, density and the position of nerves and the sinus. This is where the plan is made — guesswork at this stage is where implants go wrong.
  2. Placement. Under local anaesthetic, the implant is positioned, often with a surgical guide for precision. Most patients are surprised how comfortable this is.
  3. Healing. Osseointegration over roughly 3 months. Some cases support an immediate temporary tooth; the definitive crown waits for confirmed stability.
  4. Restoration. An impression or digital scan is taken, and the final crown — ideally made in an in-house laboratory for fit and shade control — is fitted.

Where there is not enough bone, a bone graft or sinus lift may be needed first. What types of implant treatment are there?

  • Single implant + crown: for one missing tooth, without grinding down the neighbours as a bridge would.
  • Implant bridge: two or more implants spanning several missing teeth.
  • All-on-4 / All-on-6: a full fixed arch of teeth on four or six implants for those who have lost most or all of a jaw.
  • Implant-retained dentures (overdentures): a removable denture that clips onto implants for stability.
