The biggest differences

Standard has a 2.8 mm machined neck intended to sit through the gum so the implant can heal uncovered in a single-stage procedure. Standard Plus has a 1.8 mm neck, which Straumann describes as giving more flexible coronoapical placement and additional options in the aesthetically demanding anterior maxilla. That single millimetre is very nearly the whole distinction between the two lines.

Where they are alike

Both are parallel-walled tissue-level implants in 3.3, 4.1 and 4.8 mm diameters, with a 1 mm thread pitch at 3.3 mm and 1.25 mm elsewhere. Both are offered in Roxolid with SLActive or SLA, or in titanium with SLA. Both use synOcta, an internal Morse taper with an 8 degree cone and an octagon for repositioning prosthetic parts, on Regular Neck and Wide Neck platforms.

Design

Standard reaches slightly longer in the narrower diameters — 8 to 16 mm at 3.3 mm and 6 to 16 mm at 4.1 mm — while the Standard Plus rows in Straumann's tables run to 14 mm. Straumann also lists a separate 4 mm Standard Plus Short implant for open-end situations in severely atrophic mandibles, which combines the Standard Plus neck with a Bone Level thread and is a distinct product rather than a short version of the usual implant.

Prosthetic ecosystem

The exception is the 3.3 mm Standard Plus Narrow Neck CrossFit implant, which carries a CrossFit connection instead of synOcta and its own NNC component range. A prescription that says only 'Standard Plus' can therefore produce the wrong parts. Straumann also notes that this implant's body and thread design match the 3.3 mm Bone Level NC implant, which is a further reason to record the full designation rather than the family name.

What may influence clinical selection

Where the gum is thin and the site is visible, the shorter 1.8 mm collar reduces the risk of polished metal showing at the margin, which is the reason Standard Plus exists. Where the site is posterior and single-stage healing is planned, the taller collar of the Standard implant does exactly what it was designed to do. Neither is a newer or better implant than the other; they are two neck heights of the same design.

So which is better?

Neither is universally better, and any page telling you otherwise is selling something. These are different designs with different trade-offs, and the right choice depends on the bone available, the position in the mouth, the planned restoration, and what the treating clinician is equipped and experienced to place and to service years from now. That last point is easy to overlook and matters more than most specification differences: components need to remain obtainable for as long as the implant is in place.
